Client Policies: Cancellation, Refund & Medication Info
1. Cancellation & Rescheduling Policy
We require a minimum of 24 hours’ notice for appointment cancellations or rescheduling. To secure your appointment time, all required patient intake forms must be completed at least 24 hours in advance or response of a confirmation text.
Appointment Confirmation & Fees:
If your forms are incomplete or you do not confirm your appointment as requested, your appointment may be administratively canceled to accommodate other patients.
Failure to provide at least 24 hours’ notice for cancellation, or failure to show up for a confirmed appointment ("No-Show"), will result in a no-show fee of [$25 USD].
This fee may be required prior to scheduling any future appointments.
Payment for Services: Please note that payment for all medical services and products is collected at the time of your in-office visit or at the end of your telehealth visit (via secure online payment link/Client Portal)
2. Refund Policy
Services & Consultations: Due to the customized clinical nature of our medical consultations and the immediate allocation of provider resources, all service fees are final and non-refundable once the service has been rendered.
Medications & Products: Prescription medications are dispensed by our partner licensed compounding pharmacies. All medication sales are final. Unopened, non-prescription supplements may be eligible for a store credit within 30 days of purchase, subject to approval. Shipping costs are non-refundable.
3. Medication Fulfillment, Shipping & Patient Responsibility
Medication fulfillment and shipping are managed directly by our partner, licensed and certified compounding pharmacies, which operate under strict state and federal regulations.
Pharmacy Responsibility: The selected pharmacy is responsible for the accurate dispensing, labeling, and timely shipment of your medication to the address you provide.
Patient Responsibility: You are responsible for providing a secure and accurate shipping address, being available to accept delivery, and retrieving medications promptly upon delivery. We are not liable for delays, missed deliveries, or issues arising once the pharmacy has shipped the medication.
No Guaranteed Results: Individual results from any treatment program vary significantly based on a multitude of biological and lifestyle factors. We do not guarantee any specific medical or weight loss outcome.
