Video Visits

One 30-minute video visit per quarter is included.

No. Use it or lose it. But you can text me anytime for questions that do not require a face-to-face visit.

If I determine I need to see you on video to safely answer your question, that visit counts as your quarterly visit or a $25 add-on. I will tell you before we schedule it.

What You Can Treat

Common colds, sinus infections, UTIs, skin rashes (with photos), allergies, high blood pressure, high cholesterol, diabetes, thyroid disorders, anxiety, depression, insomnia, preventative care, medication refills, and lab result reviews.

Anything requiring a physical exam (listening to lungs, looking in ears, abdominal exam), procedures, stitches, casting, emergencies, chest pain, shortness of breath, severe bleeding, head injury, or controlled substance prescriptions in most states. Please call “911” for Emergencies.

I will refer you to a local in-person provider, urgent care, or emergency room. The referral is included. Their visit is not.
Prescriptions

Do you write prescriptions?
Yes, prescription writing is included in your membership. Examples include:
Routine maintenance medications (blood pressure, cholesterol, thyroid, diabetes)
Antibiotics (within state prescribing guidelines and after appropriate evaluation)
Infection/STI treatment (after evaluation and according to CDC/state guidelines)
Medications for common acute issues (nausea, UTIs, rashes, allergies)
All prescription requests are evaluated on a case-by-case basis. I do not write prescriptions for controlled substances (Adderall, Xanax, Ambien, opioids) in a virtual-only setting, nor do I prescribe anything without a clinical evaluation first.

Any pharmacy you choose. I send electronic prescriptions directly to them.

You do, at the pharmacy. I write the prescription. You pay for the pills.

Yes. I can send to Cost Plus Drugs, Mark Cuban's pharmacy, GoodRx, or any local pharmacy offering cash discounts.

Generally, no, especially for new patients. Most states require an in-person visit for controlled substances. Since I am virtual only, I typically cannot prescribe these. Ask me about your specific situation. We can discuss appropriate referrals.
Labs

No. I order the labs. You pay the lab directly. This keeps your monthly fee low and prevents surprise costs for me.

Yes. If you have insurance, you can use it at the lab. Just give them your insurance card. I am not in-network yet, but the lab may be.

Talk to me. I can help prioritize only essential tests, find the cheapest local option, or order only what is absolutely necessary.
Emergencies & Limitations

Chest pain, severe bleeding, difficulty breathing, sudden numbness/weakness, head injury, suicidal thoughts, severe allergic reaction, or anything you would normally call 911 for.

Call 911 immediately. Do not text me. Do not wait for me to respond.

Yes. Once you are stable, text me and I will help coordinate follow-up care, review discharge instructions, and adjust medications as needed.

No. I am your virtual primary care partner, not an emergency provider. You are responsible for seeking emergency care when needed.
Insurance & Credentialing

No. DPC is a cash membership. Insurance does not pay for my monthly fee.

Almost. I am in the process of becoming credentialed in Nevada and Ohio for several plans. Once completed, you can use your insurance for labs, specialists, and hospitals—but my DPC membership remains separate.

Not for DPC services. If you want to use insurance for labs or referrals, you will handle that directly with the lab or specialist.
Getting Started

Click the "Apply for Membership" button. Fill out a brief intake form. I will review and schedule your first video visit within one week.

No. Just the first month's $99 payment when you are approved.

We meet via video for 30-45 minutes. I take a full medical history, review your current medications, discuss your health goals, and set up a plan. You will also sign a DPC membership agreement.

Not immediately. I will ask for relevant records from your previous provider, accordingly. If you do not have records, we start fresh.

You can cancel anytime. There are no refunds for partial months. When you cancel, your membership remains active until the end of your current 30-day paid period, then it ends. You will not be charged again.
Example: You pay on June 1st and cancel on June 15th. Your membership continues through June 30th. No July charge.
If you cancel, you will lose access to texting and video visits after your final day. For urgent needs after cancellation, please seek care locally.
